Please Note: This form is NOT a fixture cancellation form. 
To cancel a fixture you must fill out either the 21 day Cancelation form or the Game Cancellation form, on the Managers Portal. 

Please ensure this is submitted correctly, or the incorrect fixture maybe altered.

This can be found in a previous league email, if you cannot find your team's ID, please contact Secretary@westlancashirejfl.com .
